Step 2: You will be contacted to schedule the procedure once your information
is received.
Step 3: Print and review the instructions for preparing for a colonoscopy by
clicking this link.
Step 4: Print and complete the 3 page GI Lab patient questionnaire by
clicking this link. You will need to bring this completed form with you on
the day of the procedure.
Please be sure to complete all 4 steps.
