SCHEDULE A PROCEDURE

PLEASE NOTE: THERE WILL BE A $150 FEE FOR ANY CANCELLATIONS OR RESCHEDULING WITHIN 3 BUSINESS DAYS OF YOUR PROCEDURE

To Schedule a Colonoscopy:

STEP 1 - Provide your medical history and insurance information

Download the questionnaire and authorization form to store a credit card on file by clicking this link, fill them out, and return them along with a copy of the front and back of your insurance card by email to: schedule@cohengastro.com

or fax to our secure fax server: 312-695-4453.


STEP 2 - Schedule
You will be contacted to schedule the procedure once your information is received.

STEP 3 - Verify your insurance
When your procedure is scheduled you will receive the diagnosis codes you will need to verify insurance coverage.

For information on insurance accepted by our office and complete details about what you may owe after your visit, see the Billing and Insurance page, especially the information about our out of network status for Medicare and Blue Cross Blue Shield.

STEP 4 – Get COVID testing 2 or 3 days before your procedure (as of early 2023 this is no longer necessary unless you have symptoms)

STEP 5 - Prepare for your Colonoscopy
The prescription for your Colonoscopy Prep Kit will be called in to your pharmacy at least 2 business days prior to your Colonoscopy. Please call your pharmacy before picking it up to ensure it is ready. Do not follow the instructions that come with the Colonoscopy Prep Kit from your pharmacy. Instead, click the appropriate link below TODAY to print the Colonoscopy Instruction Packet, READ COMPLETELY, and fill out any forms in that packet which you will need to bring with you on the day of the procedure.
